Community Pharmacy Survey on Patient Safety Culture

Many pharmacies using the AHRQ Community Pharmacy Survey on Patient Safety Culture have expressed interest in comparing their results to other pharmacies. In response, AHRQ will establish the Community Pharmacy Survey on Patient Safety Culture Comparative Database. This database will be a central repository for survey data from pharmacies that have administered the AHRQ patient safety culture survey.

Purpose of the Database

  • To gather data from the AHRQ Community Pharmacy Survey on Patient Safety Culture into a central repository for comparative purposes.
  • To produce comparative results to help pharmacies identify strengths and opportunities for improvement in their patient safety culture.

Benefits of Participation and Database Products

  • Each participating pharmacy receives a feedback report comparing their results to the comparative database.
  • A free Comparative Database Report is produced providing aggregate pharmacy-level statistics. The documents do not include any information identifying participating pharmacies.
  • Until the first comparative database report is produced, pharmacies can compare their results to the 2012 Preliminary Comparative Results (PDF, 940.78 KB).
  • Additional results by pharmacy characteristics (type of pharmacy, number of affiliated locations, average number of prescriptions filled per week) and respondent characteristics (staff position and tenure in pharmacy), will also be available in the Comparative Database Report.

Definition of a Pharmacy

  • A pharmacy is a community pharmacy in a specific location.
  • Each pharmacy location that is part of a chain or health system is considered a separate pharmacy.

Participation Requirements

To submit data to the database:
  • Pharmacies must be located in the United States or in a U.S. territory.
  • Each pharmacy must have at least three completed surveys.
  • The survey must have been administered in its entirety without significant modifications or deletions:
    • No changes to any of the survey item text and response options.
    • No reordering of survey items.
    • Questions added only at the end of the survey after Section E, before the background questions in Section F.

Timeline and Activities

Registration and Data Submission

Note: Data submission is only open September 15 to November 1 in 2014 and 2016. There is no data submission in 2015 or 2017.

  • Step 1: Pharmacies, systems/chains, or survey vendors that have administered the AHRQ Community Pharmacy Survey on Patient Safety Culture indicate their interest in participating in the database by sending an E-mail with "Community Pharmacy Database" in the subject line to DatabasesOnSafetyCulture@westat.com.
  • Step 2: Database staff send registration information to interested submitters.
  • Step 3: Community pharmacies, systems/chains, or vendors submitting on behalf of pharmacies complete registration information.
  • Step 4: Database staff review the registration information and request a copy of the survey(s) actually administered. Interested submitters are notified regarding their eligibility for participation.
  • Step 5: Eligible pharmacies, systems/chains, or vendors:
    1. Should follow these survey data specifications (PDF, 167 KB; Plugin Software Help) for formatting respondent-level data files for submission.
    2. Provide pharmacy NPI number and pharmacy characteristics such as pharmacy type, number of locations, and number of prescriptions per week. Follow these specifications to gather pharmacy characteristics (PDF, 207 KB).
    3. Sign a Data Use Agreement (PDF, 187 KB) which assures pharmacies about the confidentiality of their data and explains how their data will be used.
  • Step 6: Eligible pharmacies, systems/chains, or vendors submit respondent-level survey data and other required information about their pharmacies.
    1. Data are submitted using a secure, Web-based system.
    2. Data files must meet data specifications.


The deadline for submission of all information and survey data files for any given submission year is November 1.

Data Processing and Comparative Database Report Produced

  • The data submitted are processed, and averages, standard deviations, minimum and maximum scores, and percentiles are produced.
  • Additional results by pharmacy characteristics such as pharmacy type, number of locations, number of prescriptions per week, and respondent characteristics such as staff positions and tenure in pharmacy are produced.
